Dental Summary of Benefits Coverage (SBC)
The Affordable Care Act (or health care reform law) requires that all members receive a Summary of Benefits Coverage (SBC). This document, provided by Anthem Blue Cross/ Blue Shield, may be downloaded below or a hard copy requested by emailing HR@sscc.edu.
